It is very annoying going to a doctor's appointment and having to write down all your medical information. How much easier would it be to take the time now and create a document with all your relevant information? Make sure to include your medication list and the dosage. The list needs to include all of your supplements in addition to any prescribed medication. This document should also include your major medical issues, allergies, current doctors, pharmacy, and an emergency contact. Preparing this document shouldn't take a long time. Print it out and put a copy in your wallet or your purse. Remember to update it after every doctor's appointment. This way you have a handy medical reference that you can refer to and not wrack your brains every time you need to fill out a medical questionnaire.
I encourage you to keep a copy of this information on you at all times. A good place to store it is in your wallet behind your driver's license. Also, keep an updated list in the glove compartment of your car. Make sure any major medical issues (such as diabetes or if you're are on a blood thinner) are in bold. If you are ever in an accident and are unresponsive, the police will check your wallet and your glove compartment. This handy medical information could save your life.
